Bring these in with you or send them via email to email@example.com. If you’d like to speed up the insurance verification process, please complete Step One. We are in-network providers for Blue Cross & Blue Shield, United Healthcare and Verity Network. If you are are out-of-network, we offer a time of service discount that is due at the time of service. We can prepare your superbill and guide you on how to self file, it’s easy!
STEP TWO: Email
Once you submit your demographic information, you’ll receive an email with instructions on how to create your HIPPA compliant account through our Patient Portal. The email will also contain your activation key. This patient portal is the most secure way we can send you information about your condition, treatment and billing. It is imperative that you create this account.
